Living the Mission Awards nomination deadline is January 31, 2025!

IEHP's Living the MIssion Awards honor Providers, community partners and entities whose actions and contributions embody IEHP's Mission, Vision and Values. The nomination window opens each year from November 1, 2024 through January 31, 2025.

 

Submit nominations via the nomination form, the QR code visible on the nomination form, or by visiting the Living the Mission Awards webpage.
